In Samoa, the fatalities from the measles outbreak continue to escalate, now reaching a sad total of 70 lives lost.

Samoa is currently grappling with a severe measles outbreak, with over 4,600 cases and 70 deaths reported as of December 9. This outbreak, unlike those in neighbouring Pacific nations such as Tonga, Fiji, and American Samoa, has proven more difficult to contain.

The root cause of this discrepancy lies in Samoa's significantly lower immunization coverage and higher vaccine hesitancy. According to recent data, national MMR (measles, mumps, rubella) vaccine coverage in Samoa fell to around 30% during the 2019 outbreak, creating large pockets of susceptibility and leading to the current crisis. In contrast, Tonga, Fiji, and American Samoa have maintained higher vaccination rates or more effective immunization efforts, enabling them to better contain outbreaks.

Vaccine hesitancy in Samoa is fueled by misinformation about vaccine safety, religious and philosophical exemptions, and disruptions caused by the COVID-19 pandemic. These factors have lowered trust and vaccine uptake, contributing to the outbreak's severity. Delayed and limited public health responses, including slower countering of misinformation and resource constraints, have also hampered emergency vaccination campaigns.

The demographic profile of Samoa, with many children under five who are highly vulnerable to measles complications, has further magnified the outbreak's impact.

Last week, the government of Samoa launched a massive immunization drive, aiming to vaccinate its entire 200,000-strong population. Preliminary results indicate that 90% of the population has been immunized, up from around 30% before the epidemic began in mid-October. However, it is too early to say whether the immunization drive has been effective in containing the outbreak, as its peak has not yet been reached.

Currently, 229 people are in the hospital due to measles in Samoa, with 16 critically ill children among them. The outbreak has resulted in the death of 61 children aged four or under.

In contrast, outbreaks in Tonga, Fiji, and American Samoa have been easier to contain due to higher immunization rates, with no reported deaths. The critical difference lies not in the mere existence of immunization drives but in their effectiveness, public acceptance, and sustained high coverage—factors more favorable in Tonga, Fiji, and American Samoa than in Samoa.

It is important to note that the vaccine takes 10-14 days to become effective. As health authorities attribute the low immunization rate in Samoa to the spread of anti-vaxxer conspiracy theories, the ongoing immunization drive and the vaccine's delayed effectiveness underscore the importance of continued efforts to combat vaccine hesitancy and promote public health.
