Communication via speech and language with the assistance of a tracheostomy

Living with a tracheostomy can be a challenging experience, but with regular maintenance and the help of speaking valves, many people are able to improve their speech and lead fulfilling lives.

A tracheostomy is a surgical procedure that creates an opening in the trachea (windpipe) to maintain the airway. Normally, with a tracheostomy, air bypasses the vocal cords, making speech difficult or impossible. However, speaking valves, such as the Passy-Muir valve, offer a solution. These one-way valves are placed on the tracheostomy tube and allow patients to inhale air through the tracheostomy, but close during exhalation. This forces the air upwards past the vocal cords and out through the mouth and nose, facilitating speech.

When using a speaking valve, patients often need to have their tracheostomy cuff deflated to tolerate the valve, as the valve increases airway resistance and breathing effort. Speaking valves are typically trialed for short periods and gradually increased as tolerated; they are not usually used overnight because of the increased breathing workload.

The improved airflow through the vocal cords helps restore the ability to speak without removing the tracheostomy tube. Regular follow-up appointments with healthcare professionals are essential to monitor progress, assess respiratory function, and determine the appropriateness of tracheostomy removal or ongoing care needs.

Speech-language pathologists can guide and support individuals with tracheostomies when it comes to speaking. During surgery, a surgeon creates an opening into the trachea and inserts a tracheostomy tube, which is then secured in place with sutures or a specialized holding device.

Tracheostomies are performed for a variety of conditions, including severe burns, obstructive sleep apnea, vocal cord paralysis, chronic lung disease, severe allergic reactions (anaphylaxis), comas, cancers of the head and neck, prolonged ventilator support, and obstruction of the airway by a foreign object.

Speaking valves promote vocal cord vibration, which is essential for generating sound and articulating words. With proper care and professional guidance, individuals can adapt to life with a tracheostomy and lead fulfilling lives. It is important to remember that each person's experience with a tracheostomy is unique, and modifications to certain activities may be necessary to ensure the protection and stability of the tracheostomy tube.
